The first step in planning an accredited CE activity is to complete the RSS Grand Rounds Planning Form
This short form requires you to submit basic information about your proposed activity. Upon receiving your activity, a CEHP staff member will follow up with you within 3 business days.
Once your proposed activity has been deemed eligible to proceed with planning, all individuals responsible for the planning of the activity will receive an email with instructions to complete the online Disclosure of Financial Relationships Form.
Please note: this form must be completed prior to an individual’s involvement in the planning of the activity.
Once all Disclosure of Financial Relationships Forms are received, the CEHP office will begin the process of identifying and mitigating any relevant financial relationships.
