Step 1
| The first step in planning an accredited CE activity is to complete the CE Activity Planning Form. This short form requires you to submit basic information about your proposed activity. Upon receiving your activity, a CEHP staff member will follow-up with you within 3 business days. |
Step 2
| Once your proposed activity has been deemed eligible to proceed with planning, all individuals responsible for the planning of the activity will receive an email with instructions to complete the online Disclosure of Financial Relationships Form. Please note: this form must be completed prior to an individual’s involvement in the planning of the activity. |
Step 3
| Once all Disclosure of Financial Relationships Forms are received, the CEHP office will begin the process of identifying and mitigating any relevant financial relationships. |
Step 4
| Once you have been notified by CEHP that the disclosure mitigation process is complete, you will be instructed to complete the CE Activity Application Form. Please note that this form requires that you complete very detailed information about your activity. We have provided the Guide to Completing the CE Activity Application to assist you as you complete the application. You can expect to receive notification of your accreditation decision within 7-10 days after the receipt of your completed CE Activity Application. |
