Gender Affirming Care 101: Providing Hormone Therapy

This module reviews important terminology, addresses medical biases, and identifies best practices for serving transgender and nonbinary patients.  Users of this module will learn about clinical guidelines for prescribing hormone therapy and will learn about practice changes they can make to ensure patients receive nondiscriminatory care. This module includes a role play demonstration of key concepts.

Target Audience

This activity is intended for physicians, nurses, pharmacists, and other healthcare professionals.

Learning Objectives

  1. Review important terminology for working with transgender and/or nonbinary patients along with how to incorporate language changes into your medical practice.
  2. Discuss underlying discrimination and medical biases related to the provision of care for patients who are transgender and/or nonbinary and how to not perpetuate harm.
  3. Discuss clinical guidelines for hormonal therapy for patients who are transgender and/or nonbinary.
  4. Practice having conversations around hormone therapy with transgender and/or nonbinary patients.
  5. Identify best practices that clinics can use to provide nondiscriminatory care, including the training of support staff.

Bio:

Dr. Grundman (he/him) received a BA in pharmacy, holds a European pharmacist license, and has a PhD in Pharmaceutical Sciences. He is the Director and faculty advisor for the online Master of Science and graduate certificate programs in Pharmaceutical Chemistry and Clinical Toxicology at the University of Florida College of Pharmacy. He is the author of “The Pharmacist as an LGBTQ+ Ally” (2020) and teaches classes on LGBTQ+ inclusivity to pharmacy students.

Shige Sakurai, MA, MBA, CPC

Director of Leadership Initiatives and Associate Director of the LGBT Equity Center at University of Maryland, College Park and Executive Education Faculty for USC’s Race and Equity Center

Bio:

Shige Sakurai (they/them) is a transgender person of color and a social justice leader with two decades of community and professional experiences. They currently serve as Director of Leadership Initiatives and Associate Director of the LGBT Equity Center at the University of Maryland, College Park as well as faculty in the Robert H. Smith School of Business; adjunct faculty for American University’s International Training and Education Program; and executive education faculty for USC’s Race and Equity Center. Sakurai is the first person in the U.S. to receive an officially nonbinary, X-marker driver’s license, the author of the MyPronouns.org website, and the founder of International Pronouns Day. Sakurai has received accolades at the University of Maryland, as well as international, national, and local awards from Diversity Abroad, the Consortium of Higher Education LGBT Resource Professionals, and the Mayor of Washington, DC.

Penny L. Jacobs, CRNP

Family Nurse Practitioner at the University of Maryland Health Center and member of the University President’s Commission for LGBT Issues

Bio:

Penny Jacobs (she/her) is a Family Nurse Practitioner at the University of Maryland Health Center. In 2009 she initiated the University’s Hormone Therapy program for Transgender students. She is the Chair of the Quality Improvement Committee and has been a member of the University President’s Commission for LGBT Issues for over 4 years. In her role she also provides health education to student and staff groups about LGBTQ+ health, sexual health, and health careers.

Dane Ray, MA

Masters of the Arts in Rehabilitation Counseling, Experienced trainer

Bio:

Dane Ray is a Washington, DC based mental health professional, who currently works with the LGBTQ population to increase self-efficacy, resilience and advocacy for transgender and non-binary people. Dane has 14 years of experience as an educator, within and on behalf of the transgender communities, with additional experience in prevention education and research related to HAHSTA (HIV/AIDS, Hepatitis, STD and TB Administration). Dane believes that in order to achieve equitable care, it is critical that mental health and community health providers be informed of and prepared to meet the needs of transgender and gender expansive people. In working towards this goal, Dane’s current research interests center on public health communication in response to gaps in the healthcare system for self-identified transgender and gender expansive people. Dane serves on the Board of Directors of the Rainbow History Project as well as sitting on the Board of Directors for the DC Mental Health Counseling Association. Dane earned a bachelor’s degree in Religion and Leadership Studies from Birmingham-Southern College in 2007. Dane earned a Master of Arts degree in Vocational Rehabilitation Counseling program at the University of the District of Columbia.

Dane is sitting for a Licensed Professional Counselor credential in DC and Maryland. Dane is a parent, spouse and avid world traveler.

Nurse Practitioner, Adult LGBTQ Clinical Lead, and Director of HIV Services at Mary's Center

Bio:

Dave (he/his) is a Nurse Practitioner, Adult LGBTQ Clinical Lead, and Director of HIV Services at Mary's Center, where he also previously served as the Chair of their Diversity Committee for 2019 and 2020. His training as a board-certified HIV Specialist included specific training on gender and sexuality topics including gender affirming care. He has over five years of experience providing gender-affirming care that is medically safe and emotionally supportive for patients, and is passionate about working with other healthcare workers to provide gender-affirming care for transgender and nonbinary individuals.

Bianca Palmisano, RN, MSN

Registered nurse, sex educator, and the owner of Intimate Health Consulting

Bio:

Bianca Palmisano (they/them) is a registered nurse, sex educator, and the owner of Intimate Health Consulting. They specialize in training healthcare providers around issues of sexual health, as well as LGBT, sex worker, and sexual assault survivor competency. They are a two- time presenter for the Philadelphia Transgender Health Conference and the Woodhull Sexual Freedom Summit, and a planning committee member for DC's Transgender Day of Remembrance. Palmisano is the primary author of "Safer Sex for Trans Bodies," an outreach and education guide for the trans community sponsored by Whitman Walker Health and the Human Rights Campaign. They are a regular guest lecturer at George Washington University, Johns Hopkins University, University of Chicago, and a variety of other medical and higher learning institutions
