Cancer Survivorship Series
Primary care providers play a critical role in providing follow-up care for cancer survivors, including dealing with many of the physical, psychological, practical, informational and spiritual challenges after the completion of cancer treatment.
Clinicians can learn about caring for survivors of adult-onset cancers through a series of ten enduring online educational modules:

- Module 5: A Team Approach: Survivorship Care Coordination
- Module 5 Companion: Advancing Patient-Centered Cancer Survivorship Care
- Module 6: Cancer Recovery and Rehabilitation
NOTE: Each lesson in the training can be accessed independently. To start, please complete the learners' survey that appears as the first activity in the training.

The Cancer Survivorship Series for Primary Care Providers is a collaboration between the American Cancer Society and the GW Cancer Center originally funded through a 5-year cooperative agreement (#5U55DP003054) with the Centers for Disease Control and Prevention (CDC). It is currently supported through a cooperative agreement (#NU58DP007539) from the CDC.

Continuing education credits (CEs) are available at no cost to physicians, nurse practitioners, physician assistants, nurses and Certified Health Education Specialists. Each module corresponds to 1 credit hour. In order to receive CEs, participants MUST complete at least one module (for 1 credit hour), including all module activities. Participants will be provided instructions to claim credit and print certificates upon module completion.

Modules 1, 2, 5, & 5.1 have been approved by The George Washington University School of Medicine and Health Sciences for AMA PRA Category 1 Credit(s)™. Physicians may claim AMA PRA Category 1 Credit™. Please check your state eligibility requirements for claiming credit. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Nurses interested in submitting to their board of nursing when the board of nursing accepts AMA PRA Category 1 credit towards their re-licensure CE requirements may claim a Non-Physician Certificate.
The George Washington University School of Medicine and Health is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.
